Hope Community Support Program

Hope is a program dedicated to helping people with mental illness live healthy, productive and independent lives.

Catholic Charities Hope Community Support Program has been serving people in Benton, Sherburne and Stearns counties since 1988. The program accepts referrals from a variety of sources including health care providers, mental health professionals, families and individuals seeking service.

Program Details

Eligibility

Mental health services are offered dependent upon the eligibility of the individuals. Contact us and we will help you determine eligibility. 

Referrals

Hope Community Support Program accepts referrals from a variety of sources, including health care providers, mental health professionals, families and individuals seeking service.

Community Support Program


Our program supports adults with mental illnesses as they cope with the challenges of daily living, assisting each client in achieving and maintaining his or her highest level of self-sufficiency.


Adults living with serious and persistent mental illness who do not receive Medical Assistance may qualify for Traditional Community Support Program Services through Catholic Charities Hope Community Support Program.

Adult Rehabilitative Mental Health Services


Adults living with mental illness or traumatic brain injury who receive Medical Assistance may qualify for the Adult Rehabilitative Mental Health Services (ARMHS) portion of Catholic Charities Hope Community Support Program’s services. These services enable the client to develop and enhance psychiatric stability, social competencies, personal and emotional adjustment, and independent living and community skills.


Our program supports adults with mental illnesses as they cope with the challenges of daily living, assisting each client in achieving and maintaining his or her highest level of self-sufficiency.

Each client is assigned an ARMHS worker, who assists with:

  • Interpersonal Communication
  • Community Resource Utilization and Integration
  • Crisis Assistance and Relapse Prevention
  • Health Care Directives
  • Budgeting and Shopping
  • Healthy Lifestyle Skills and Practices
  • Cooking and Nutrition
  • Transportation
  • Medication Monitoring and Education
  • Symptom Management
  • Household Management
  • Employment-related Skills
  • Transition to Community Living

Hope Community Support Groups

Depression & Bipolar Support Group

Join us in a safe place where people living with depression or bipolar disorder can share their experiences, concerns, and feelings. This group provides a chance to meet others with common concerns and to gather strength from one another.

Here & Now Support Group

Join us to learn a mindful approach to mental health. The group will focus on learning to take advantage of the opportunities and possibilities open to us everyday, to live in and appreciate the moment and to learn how to build on the positives in our lives.

Schizophrenia Support Group

You are not alone. Here is a safe setting in which to learn about yourself and others. A place to develop support and friendships. A chance to take advantage of education, skill-building, and mentoring activities.
